Preparing for your visit

A quick checklist of what to have ready before your consultation with Dr. Souptik Majumder. Check items off as you go, or print/download this page to take with you.

Reports & imaging

All scans (CT / MRI / PET-CT) — disc or digital copy
Biopsy and pathology reports
Blood test results, if recent
Prior discharge summaries or referral letters

Medical history

List of current medications and doses
Known allergies (medicines, food, etc.)
Other conditions being treated (diabetes, heart disease, etc.)
Names/contacts of other doctors involved in your care

Practical

A companion or family member, if you'd like one with you
Insurance details or ID, if applicable
A list of your own questions — nothing is too small to ask
Comfortable clothing (especially if imaging is planned the same day)
